Office Policies

WELCOME TO THE OFFICE

 

To help you get acquainted with the office, we have prepared a few words about our policies and fee schedules.  Please read this so that you understand the guidelines.  

 

Your Appointment:

Your appointment is time set aside for you to see the Doctor.  We have a twenty-four (24) hour cancellation policy.  If you cancel an appointment less than 24 hours prior to its scheduled time, you will be billed the entire visit fee.  A message may be left on our answering machine at any time to cancel your appointment.  The earlier you can inform us of a change in your plans, the better chance we have of accommodating another patient.  We also respect your time and make every effort to be punctual for your appointment.

 

Fragrances:

Some of our patients are allergic to perfumes and hair sprays.  We would appreciate it if you refrain from wearing these to the office.

 

Fees & Payment:

·    The office accepts Aetna, Anthem/Blue Cross Blue Shield, Humana, Medicare, Sloan's Lake/Cofinity and United Healthcare insurances.  A copay will be collected at the time of your visit.  For all other insurances, we require payment for services at the time they are provided.  We supply a standard itemized receipt (HCFA form) that you may submit to your insurance company in case you qualify for reimbursement.  We regret we are unable to accept assignment from all insurance carriers.

·    In the case of minors, the parent or guardian who brings the minor in is responsible for the bill.  If the parents are divorced or separated and one is responsible for the medical bills, we require payment from the person accompanying the minor, who can then be reimbursed by the responsible party.

·    If your check is returned from the bank, we will add a $50.00 “return check” fee to your account.

 

Children:

Children must be supervised by their caretaker and remain in the waiting area only.  Please do not allow children to roam about in the hallway or other rooms.
